إيه المقصود بـ Mastocytosis? 🧬 Definition Mastocytosis is a rare disorder. It involves abnormal proliferation and accumulation of mast cells. It can affect the skin, bone marrow, and other organs. --- 🌍 Types Cutaneous mastocytosis → confined to the skin. Systemic mastocytosis → involves internal organs (e.g., liver, spleen, bone marrow). Indolent form is most common, but aggressive forms also exist. --- 🔍 Pathophysiology Mutation in the KIT gene (most commonly D816V). This mutation causes uncontrolled activation of mast cells. Excess histamine and other mediators are released. --- 👩‍⚕️ Clinical Features Skin lesions: urticaria pigmentosa, flushing, pruritus. Systemic symptoms: abdominal pain, diarrhea, bone pain. Anaphylaxis may occur after triggers (drugs, stings, food). --- 🧪 Diagnosis Skin biopsy → shows mast cell infiltration. Bone marrow biopsy if systemic involvement is suspected. Serum tryptase level is usually elevated. Genetic testing for KIT mutation can confirm. --- 💊 Treatment Avoidance of triggers (alcohol, NSAIDs, opioids). Antihistamines for symptom control (H1 + H2 blockers). Corticosteroids or leukotriene antagonists may be used. Severe cases: tyrosine kinase inhibitors (e.g., midostaurin). --- ⚠️ Complications Recurrent anaphylaxis. Osteoporosis due to mast cell mediators. It causes degeneration of motor neurons in the spinal cord and brainstem. This leads to progressive muscle weakness and atrophy. 🧬 Cause SMA is most commonly caused by a mutation in the SMN1 gene. This gene is responsible for producing survival motor neuron (SMN) protein. Lack of this protein leads to motor neuron death. 👶 Types There are several types of SMA, classified by age of onset and severity. Type 1 (Werdnig-Hoffmann) is the most severe and appears in infancy. Type 2 and Type 3 have a later onset and milder progression. Type 4 is adult-onset and usually the mildest. 🧓 Symptoms Muscle weakness, especially in the legs and trunk, is a common symptom. Affected individuals may have difficulty sitting, standing, or walking. Breathing and swallowing problems can also occur in severe cases. 🧪 Diagnosis SMA is diagnosed through genetic testing to identify SMN1 mutations. Electromyography (EMG) and muscle biopsy may also support diagnosis. Newborn screening is increasingly used in many countries. 💉 Treatment There is no cure, but disease-modifying treatments are available. Nusinersen (Spinraza) is an intrathecal injection that increases SMN protein. Onasemnogene abeparvovec (Zolgensma) is a gene therapy for young children. Risdiplam (Evrysdi) is an oral medication approved for various SMA types. 🧘 Supportive Care Physical therapy helps maintain muscle function. Respiratory support may be needed in advanced stages. Nutritional support and orthopedic care improve quality of life. 📊 Prognosis Prognosis varies widely depending on SMA type and treatment. Early diagnosis and intervention significantly improve outcomes.
